General Health Assessment & Screening Services
The purpose of the General Health Assessment/Screening services is to provide preliminary data for screening of participant's risk for heart disease, stroke, diabetes and cancer. Written consent and results for participant indicates that the assessment does not constitute a diagnosis and is not intended to replace continuing care for chronic disease. Participants are strongly encouraged to see their licensed health care provider source and/or physician for further evaluation and care.
